Appointments required for specific LBJ services

Pago Pago, AMERICAN SAMOA — As the territory continues under Code Red, prompting the closure of government services, except for essential ones, members of the public have raised questions on how they are to obtain specific documents from the LBJ Medical Center — such as birth certificates, death certificates, copies of medical records and other information.

In a notice, LBJ said an appointment is required for these services.

To request copies of medical records or release of medical information, call 633-1222 ext. 676 for an appointment. Furthermore, call 633-1222 ext. 678 for an appointment to request or pick up birth or death certificates.

Requests can also be made via email:  <[email protected]> and make sure to include the type of request and current phone number to contact regarding the request and to schedule an appointment.

“There will be NO WALK-INS,” said the notice, and reiterated calling or emailing to schedule an appointment and additional information will be provided by staff prior to the appointment.

At the hospital, enter at the Emergency Room entrance area and security will direct you to the Clinics Screening Tent (#5) for a COVID-19 Test. Everyone will be tested for COVID-19 before entering the hospital. Masks and social distancing will be enforced.
